What the inspector wrote

On June 16, 2025, at 2:10 PM, Licensing Program Analyst (LPA) Sandra Pulido, arrived for the purpose of a case management visit regarding an unusual incident report (UIR) received in our office. Census was not taken due to the children being off for the summer. The UIR was received by the licensing agency on 6/5/25. It indicates that child #1 (C1) was rolling on the playground grass and rolled on the metal grid panel that covers the electrical, resulting in a red mark on the back of their head. LPA toured the playground area where the child was injured and took photos. LPA was informed that C1 rolled onto the smaller metal panel that measures 3.5\342\200\231 x 2.5\342\200\231. Based on information gathered, observations, and interview with Site Manager Wiedensohler, the facility acted appropriately, and no violations have been identified. Staff immediately treated the child for injuries by providing an icepack, called the parents and child was picked up from facility to be taken to urgent care in which it was determined only observation was needed; due to an all-staff day on 6/6/25, C1 returned to school on 6/9/25. A work order was placed the same day of the incident to sand down and smooth the metal panels, which was completed the following week. Another work order was placed to add rubber matts to both panels. Per Site Manager Wiedensohler, the children started their summer break as of 6/12/25, returning after Labor Day, and no other children utilize the playground in which the injury took place. An exit interview was conducted, and a copy of this report and appeal rights was provided to Site Manager, Lori Wiedensohler.
