What the inspector wrote

On September 16, 2025, at 11:00 a.m., Licensing Program Analyst (LPA) Sandra Pulido and Licensing Program Manager (LPM) Pauline Beschorner conducted an unannounced case management visit and met with Site Manager Lori Wiedensohler. The visit was prompted by an Unusual Incident Report received by the department on September 10, 2025, regarding a child who was accidentally left in a classroom for less than one minute. During the investigation, LPA Pulido interviewed three staff members. It was disclosed that the teachers were preparing to take the children outside when Staff 1 (S1) counted 13 children. As the group exited the room, Child 1 (C1) became distracted. Staff 3 (S3) initially confirmed that all children had exited, but upon recounting, Staff 2 (S2) noticed C1 was missing. S2 immediately opened the door and found C1 sitting at the table. Based on staff interviews and a review of records, it was determined that the staff followed all appropriate procedures in reporting the incident, and no violations were cited during the visit. In addition to investigating the incident, LPA and LPM reviewed and measured the newly added Room 8, which is designated as a preschool classroom for children ages 2 to 5 enrolled in the Head Start Program. Observations revealed no health or safety concerns. The licensee is not requesting an increase in overall capacity, but rather the formal addition of Room 8 to the facility\342\200\231s license. LPA will update the license accordingly and mail the revised license to the licensee upon completion. An exit interview was conducted, and a copy of the report was provided to Site Managers Lori Wiedensohler and Tanya Carter.
