What the inspector wrote

On date and time listed, Licensing Program Analysts (LPAs) Perla Ordonez and Taityana Beson arrived at the facility to conduct an annual inspection as part of a compliance review. LPAs toured the facility, inside and out, records were reviewed, and the following was observed and/or discussed: Normal days and hours of operation are: Monday \342\200\223 Friday; 06:30AM \342\200\223 05:00PM. OFF-LIMIT AREAS INCLUDE: Bedroom 1, bedroom 2, laundry room, backyard, and garage. The facility is operating within the licensed capacity and appropriate ratios. \302\267 Appropriate supervision provided during this inspection. \302\267 A working telephone is present and current number on file. \302\267 Appropriate fire extinguisher, smoke detector and carbon monoxide detector present and were tested by the Licensee during this inspection. \302\267 All hazardous items are not stored inaccessible to children. During facility tour, LPA Benson observed knives in an accessible drawer in the on-limits kitchen and LPAs observed children went into the kitchen where their water bottles were located . \302\267 Toxins are locked. \302\267 Weapons are not present per Licensee Josephine Galindo. Licensee understands all guns, weapons and ammunition must be key locked separately and made inaccessible per Title 22 Regulations. \302\267 This is a one story home. \302\267 Verification of control of property on file. \302\267 Property Owner/Landlord Consent (LIC 9149)/Notification (LIC 9151) on file. \302\267 Facility Sketch, Emergency Disaster Plan & Notification of Parent\342\200\231s Rights poster are posted. \302\267 Mandated Reporter Child Care Providers Training expired on 05/2025. \302\267 EMSA Pediatric CPR and First Aid Card expires on 06/2026. \302\267 Health & Safety Certificate \342\200\223 completed. \302\267 No bodies of water at this time. Licensee understands all bodies of water including ponds, above ground pools & spas, in-ground pools & spas, and some fountains must be properly covered or fenced per Title 22 Regulations. The Department must be notified before and after installation of the above types of bodies of water. In addition, all wading pools or similar product must be emptied immediately after use and stored in an upright position. \302\267 Clean, safe and age appropriate toys and play equipment not present. During facility tour, LPA Benson observed play equipment which was not in good repair as the wooden boarder was broken and splintered, but LPAs observed that children were not playing in that area during the visit. \302\267 Current roster not on file. During record review, LPAs observed that roster was not up-to-date as C1 and C2 were missing from the current roster and children who Licensee stated no longer attended did not have the "Date Left" section filled out. \302\267 Documentation of fire and disaster drills not on file. During record review, Licensee could not provide LPAs with documentation of fire/ disaster drills. \302\267 Children\342\200\231s records are not complete. During record review, LPAs observed that C1 was missing proof of updated immunizations. Additionally, LPAs observed that C3 was missing proof of the Consent For Emergency Medical Treatment (LIC627). \302\267 Employee\342\200\231s records are not complete. During record review, LPAs did not observe proof of measles immunization for S1. Additionally, LPAs observed that Licensee and S1 did not have proof of Tuberculosis (TB) Clearance on file. \302\267 The Licensee was informed of their reporting requirements and is provided with the Regional Office\342\200\231s Unusual Incident Reporting email mailbox: UnusualIncidentReportsDO09@dss.ca.gov \302\267 Resident and/or staff records reviewed on 08/13/2025 indicate that all adults who require caregiver background checks have received all required clearances or exemptions. \302\267 The Licensee can submit transfer forms to associate new individuals or to disassociate someone from your facility at: Associations_Disassociations862@dss.ca.gov LPAs discussed the safe sleep regulations with Licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPAs also informed Licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/ (800) 514-0383 (TTY) and link to publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: https://www.ada.gov/resources/child-care-centers/ Licensee was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. LPA Perla Ordonez and Taityana Benson informed Licensee Josephine Galindo that this report dated 08/13/2025 document(s) one Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA Perla Ordonez and Taityana Benson informed the Licensee Josephine Galindo to provide a copy of this licensing report dated 08/13/2025 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Technical Support Program (TSP) brochure was provided to Licensee during this visit. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. During the exit interview, the LICENSEE Josephine Galindo, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. Exit interview conducted and report was reviewed with the Licensee Josephine Galindo.
