What the inspector wrote

On 01/08/2026 at approximately 10:52AM, Licensing Program Analyst (LPA) Jesse Gardner was at the facility investigating complaint #10-CC-20260107095914. In relation to the complaint, LPA found a violation of Title 22 where the facility did not report an incident within the parameters of the regulation that requires the facility to report incidents within 24 hours via telephone or fax, and in writing within 7 days. LPA met with Director informed them of the purpose of this visit. An exit interview was held where a copy of this report was reviewed and provided along with copies of the LIC809-D (deficiency page), and Appeal Rights, along with a Notice of Site Visit.

  • REPORTING REQUIREMENTS

    101212 - Reporting Requirements:(d) Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. In addition, a written report containing the information specified in (d)(2) below shall be submitted to the Department within seven days following the occurrence of such event.(1) Events reported shall include the following:(C) Any unusual incident or child absence that threatens the physical or emotional health or safety of any child. This requirement was not made as evidenced by: Based on evidence obtained, the Licensee did not comply with the section cited above where C1 eloped from the facility and the incident was not reported to CCL in a timely manner. This poses a potential health and safety risk to children in care.
