LPA interviewed 3 staff, the Director, and a prior staff, reviewed camera footage, and reviewed other evidence in relation to the incident. 3 of 3 staff interview’s revealed supervision was maintained the duration of C1’s departure of the classroom. This was also confirmed by video footage review. Based on the information obtained from interviews, the allegation was found to be Unsubstantiated. A finding of Unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur. An exit interview was conducted, and a copy of this report was provided along with copies of the confidential names list, of the Appeal Rights was provided. A Notice of Site visit was given, and the Licensee understands that it must remain posted for 30 days.
