What the inspector wrote

On Thursday, March 5, 2026 Licensing Program Analyst (LPA) Jesse Gardner conducted an unannounced Case Management inspection to follow-up on information reported to Community Care Licensing (CCL) on February 27, 2026 via the Duty Officer. LPA met with Director Marie Markham and toured the facility. On February 27, 2026, CCL received information via phone that on February 26, 2026, Child One (C1) had been left behind in a classroom when their class transitioned to the playground. C1 was left alone inside the classroom for approximately 6 minutes until another staff found them. Records review confirmed that the CCC was operating within proper staff to child ratios; however, the child was inadvertently left behind. Additionally, the involved staff was relieved of their duties. The child's authorized representative was notified at pickup. Based on information gathered, a violation of Title 22 was identified and the facility was issued a citation. An exit interview was held with Director Marie Markham. A copy of this report was issued, along with a Notice of Site visit and copies of the LIC809-D, and Appeal Rights were provided. This report shall be public record for three years.

    101229 Responsibility for Providing Care and Supervision: (a) The licensee shall provide care and supervision as necessary to meet the children's needs.(1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement was not met as evidenced by: Based on Director's investigation, and video footage, C1 was left in the classroom alone without supervision. This poses a potential health and safety risk to children in care.
