On April 22, 2026 at approximately 11:17AM, Licensing Program Analyst (LPA) Jesse Gardner conducted a case management visit in response to the receipt of an unusual incident report (UIR) from the facility. The UIR was received by the licensing agency on April 14, 2026. It indicates Child One (C1) had been bothered by Child Two (C2) on a daily basis, and physically assaulted by Child Three (C3). On today's visit, LPA interviewed staff, and C1, C2, and C3. Interviews with the children revealed inconsistent statements; however, as a precaution, the Director moved C2 to another classroom. Based on information gathered, the facility acted appropriately and no violations have been identified. Parents of all children involved were notified of the allegations, and the relocation of C2 to another classroom. An exit interview was conducted and a copy of this report was provided to facility staff.
