What the inspector wrote

Licensing Program Analyst (LPA) Samuel Lopez arrived at the facility to conduct a case management visit in response to the receipt of an unusual incident report (UIR). The UIR was received by the licensing agency, via phone, on 12/10/2025 and the documentation (LIC 624) was received on 12/11/2025. The UIR documented an alleged Personal Rights violation. Upon arrival this date on 12/15/2025, LPA Lopez met with facility Site Supervisor Jessica Dougherty and ECE Coordinator Linda Rodriguez and stated the purpose of the visit. Records were reviewed/obtained and interviews were conducted. Based on the information gathered on this initial visit, the facility complied with the regulation section of reporting requirements. Any other regulation requirements were not addressed at this time. A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with the Site Supervisor Jessica Dougherty and ECE Coordinator Linda Rodriguez.
