What the inspector wrote

Four out of five staff interviews confirmed that infants are frequently rotated between cribs based on nap schedules. All five staff interviewed acknowledged they were unaware that only infants capable of climbing out of a crib may transition to a sleeping mat. Based on staff interviews, direct observations, and a review of facility records, the preponderance of evidence supports the allegation. Therefore, the allegation is substantiated. A violation of California Code of Regulations, Title 22, Section 101430(3)(E) regarding safe sleeping practices is cited on the attached LIC 9099D. An exit interview was conducted with the licensee. LPA provided a copy of this report, appeal rights, and a Notice of Site Visit (NOS). The licensee acknowledged that the NOS must remain posted in a prominent location for 30 consecutive days, clearly visible to families and caregivers.
