This is an amended report to a visit conducted on July 30, 2025. On September 16, 2025, at 1:01 PM, Licensing Program Analyst (LPA) William Chancellor made an unannounced Case Management visit at Rancho Christian Preschool to deliver an amended report. LPA met with Director Erin Fonda to correct an error on the previous report. The amended 809 now correctly reflects that, there is not sufficient evidence to corroborate that child 1 (C1) injury was caused by staff 3 (S3). It must be acknowledged that S2 was not involved with the incident and S3 is no longer associated to the facility. Facility was toured and census was taken. No deficiencies are being cited at this time. An exit interview was conducted, signatures were obtained for the amended pages and a copy of this report was provided to Director Erin Fonda. A notice of site visit was also provided and facility representative was reminded the notice must be posted for 30 consecutive days.
