Inspector’s narrative

What the inspector wrote

On 10/28/25, Licensing Program Analysts (LPA), Kelli Waters and Sumayya Habeebulla, conducted an unannounced visit to the facility for a separate unrelated issue. LPAs met with Director, Yasmin Rawlins, conducted a tour of facility, interviewed Licensee and reviewed records. During record review of 6 staff, LPAs observed that multiple staff’s files were incomplete. LPAs found that 4 out of 6 staff were missing immunization records, 2 out of 6 staff were missing Tuberculosis testing verification, and 3 out of 6 staff were missing a health screening. In addition, 2 out of 6 staff reviewed did not have the required AB 1207 training component for mandated reporting. After requesting the items stated above, the facility was unable to produce the required staff documentation at the time of visit. Licensee will be cited under California Code of Regulations, Title 22, Personnel Records 101217(a) and Health and Safety Code, 1596.8662(b)(1) and 1596.7995(a)(1). See 809-D for cited deficiencies. An exit interview was conducted, and a copy of this report and appeal rights were provided. A copy of this report must be made available to the public, at the facility site, for 3 years.

    (a) The licensee shall ensure that each child is accorded the following personal rights:(1) To be accorded dignity in his/her personal relationships with staff and other persons.This requirement is not met as evidenced by: Based on interviews, the licensee did not comply with the section cited above in ensuring staff are not spoken to in a disrespectful manner in the presence of children which poses a potential health, safety or personal rights risk to persons in care.

  • ARTICLE 1. General Provisions and Definitions

    a) (1) Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles...This requirement has not been met as evidenced by: Based on record review, 4 out of 6 staff records reviewed did not have proof of immunizations to measles or pertussis, therefore the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

  • ARTICLE 2. Administration of Child Day Care Licensing

    (b) (1)   On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training.This requirement has not been met as evidenced by: Based on record review, 2 out of 6 staff had only completed general training which does not meet the AB 1207 requirement, therefore the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.
