On 02/20/2025 at 11:50 am, Licensing Program Analyst (LPA) Gabriela Hernandez arrived at the facility to conduct an annual inspection as part of a compliance review. LPA toured the facility, inside and out, records were reviewed, and the following was observed and/or discussed: \302\267 Normal days and hours of operation are: Monday thru Friday from 7:30 am to 5:00 pm. \302\267 Off-limit areas include: left side of backyard, all bedrooms, restroom in hallway and garage \302\267 The facility is licensed to have no more than 14 children as a large FCCH and is operating within the licensed capacity and appropriate ratios. There were 6 children present with Licensee. \302\267 Appropriate supervision was being provided during this inspection \302\267 A working telephone is present, and the current phone number /email are on file \302\267 A fully charged fire extinguisher (2A:10BC) was observed. A smoke detector and carbon monoxide detector were present and tested by the Licensee during this inspection. \302\267 Electric fireplace is properly screened to prevent access by children. \302\267 All hazardous items are stored inaccessible to children. \302\267 Toxins are locked and inaccessible to children in care. \302\267 Weapons are not present per Licensee. Licensee understands all guns, weapons and ammunition must be key locked separately and made inaccessible per Title 22 Regulations \302\267 Single Story Home; no stairs to barricade. \302\267 Clean, safe, and age-appropriate toys are provided \302\267 Current roster on file \302\267 Facility Sketch, Emergency Disaster Plan and Notification of Parent\342\200\231s Rights poster are posted \302\267 Documentation of fire and disaster drills are on file \342\200\223 Last drill was conducted on 02/14/2025. \302\267 LPA observed a cold plunge spa in the backyard under the porch that had a cover on the spa with 4 side release buckles, however, the cover did not have any lock; therefore accessible to children in care. Type A deficiency issued. Licensee understands all bodies of water including ponds, above ground pools & spas, in-ground pools & spas, and some fountains must be properly covered or fenced per Title 22 Regulations. The Department must be notified before and after installation of the above types of bodies of water. In addition, all wading pools or similar product must be emptied immediately after use and stored in an upright position. \302\267 Verification of control of property is on file \302\267 Children\342\200\231s records are not complete. \302\267 Mandated Reporter Training expires 03/2025. \302\267 Pediatric CPR and First Aid Card expired on 02/07/2025. \302\267 Health & Safety Certificate \342\200\223 completed. \302\267 Resident and/or staff records were reviewed and all adults who require caregiver background checks have received all required clearances and/or exemptions. Licensee Elizabeth Cota was reminded that all adults 18 and over, living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. LPA discussed the safe sleep regulations with Licensee Elizabeth Cota and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP . When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. On-line Licensing forms & regulations for a Child Care Center can be obtained on the Department\342\200\231s website: www.ccld.ca.gov . Additionally, there is a link to \342\200\234Receive Important Updates\342\200\235 located on the right side of the page, immediately above Quick Links. One can add their email address and choose which program(s) they wish to receive Provider Information Notices (PIN) for. The Duty Officer is available to answer questions Monday \342\200\223 Friday; 8:00am to 5:00pm at: 951-782-4200 Licensee was informed of their reporting requirements and is provided with the Regional Office\342\200\231s Unusual Incident Reporting email mailbox: UnusualIncidentReportsDO10@dss.ca.gov Licensee can submit fingerprint transfer forms to associate new individuals or to disassociate someone from the facility at: Associations_Disassociations858@dss.ca.gov During the exit interview, Licensee Elizabeth Cota confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. If a Civil Penalty has been assessed during this inspection. Payment is due when billed and the check(s) or money orders shall be made payable to the \342\200\234California Department of Social Services\342\200\235. YOU WILL RECEIVE AN INVOICE IN THE MAIL. DO NOT SEND MONEY UNTIL YOU RECEIVE YOUR INVOICE. DO NOT SEND CASH. See LIC809-D for cited deficiencies. LPA informed licensee that this report dated 02/20/2025 documents 1 Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Also, LPA informed the licensee Elizabeth Cota to provide a copy of this licensing report dated 02/202/2025 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. An exit interview was conducted, and this report was reviewed with Licensee Elizabeth Cota. Appeal rights were discussed and provided during the exit interview.
