Inspector’s narrative

What the inspector wrote

On 07/01/25, Licensing Program Analyst (LPA), Kelli Waters, conducted an unannounced visit to the facility for a separate unrelated issue. During the Case Management visit, LPA met with Director, Katrina Wanemacher and Assistant Director, discussed the reason for the visit, took a census and toured the facility. During the inspection, LPA observed both the “infant room” for children ages 6 weeks to 12 months and the “toddler room” for children ages 12 months to 24 months and conducted record review. During the record review, three out of seven staff records were incomplete and missing required proof of immunizations. The facility will be cited under the California Health and Safety Code 1596.7995. See 809-D for deficiencies. An exit interview was conducted, a copy of this report, and appeal rights were discussed and provided to Director, Katrina Wanemacher. The Notice of Site Visit must be posted for 30 days. A copy of this report must be made available to the public, at the facility site, for 1 year.
