What the inspector wrote

On 10/22/25, Licensing Program Analysts (LPA) Kelli Waters and Sumayya Habeebulla, conducted an unannounced subsequent Case Management visit to follow up on Unusual Incident Report (UIR) that was submitted via phone to Community Care Licensing (CCLD) on 08/11/25, regarding an incident that took place on 08/08/25. LPAs met with Assistant Director, Jasmine Jackson, and informed her of the reason for the visit. On 08/11/25, the Director reported the following to CCLD; on 08/08/25, at approximately 10:20 am, the toddler classroom was transitioning from outside to inside. Upon returning to the classroom at approximately 10:23 am, staff #1 (S1) found a child (C1) that had been left on the playground unattended. During the investigation, LPA Waters inspected the facility, conducted interviews, and reviewed records. Interviews and record review revealed that C1 was left unattended on the infant playground for approximately 7-8 minutes while the toddler staff and other children were inside the classroom. According to interviews, classroom staff became aware of C1’s absence after S1 from another classroom heard C1 outside. S1 returned C1 to the classroom. The facility is currently conducting an internal investigation to determine further action needed and will review supervision training procedures with staff. This facility is being issued a TYPE A deficiency for Responsibility for Providing Care and Supervision with an immediate $500 civil penalty assessed. See 809D and LIC421IM for details. An exit interview was conducted, a copy of this report, and appeal rights were given. Exit interview conducted and report was reviewed with the Director, Katrina Wanemacher. Appeal rights were provided, and a Notice of Site Visit was given and must be posted for 30 days.

    No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation.This requirement has not been met as evidenced by: Based on interview and record review, C1 was left unattended for approximately 7-8 minutes, therefore the licensee did not comply with the section cited above which posed an immediate health, safety or personal rights risk to persons in care.
