Inspector’s narrative

What the inspector wrote

It was reported that on or about 07/29/2025 at approximately 9:32 AM, video footage showed a child being mishandled by Staff 1 (S1). According to reports, children were transitioning from indoors to outdoors when a child requested assistance with putting on their shoes. S1 approached to assist the child; however, S1 appeared visibly frustrated and handled the situation in an aggressive manner while attempting to put the child’s shoes on. The child began to cry, and S1 requested assistance. Another staff member then intervened and calmly assisted the child. LPA reviewed the video footage, which corroborated the reported incident. Additionally, interviews with relevant parties disclosed that during a different time S1 was observed dragging a child across the floor and yelling at the child, causing the child to cry. The child was subsequently removed from the situation by another staff member. During an interview, S1 acknowledged not having adequate training in managing certain behaviors. Based on footage reviewed, interviews conducted and admission from staff themselves, the Department has determined that the preponderance of evidence standard has been met. Therefore, the above allegation is found to be SUBSTANTIATED . Violations of California Code of Regulations, Title22, are being cited on the attached; see LIC 9099D for cited deficiencies. An exit interview was conducted with Site Director Claudia Gonzalez. A copy of this report, the Notice of Site Visit, and Appeal Rights were provided. The Notice of Site Visit must remain posted for 30 consecutive days. LPA Aman Lama informed the Director, Claudia Gonzalez to provide a copy of this licensing report dated April 03 , 2026, that documents any Type A citation(s) to parents/guardians of all children currently enrolled, or newly enrolled by the next business day or the next day the child(ren) is(are) in care. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification and kept on file for 12 months from the date of this report. LPA Aman Lama informed Director, Claudia Gonzalez that this report dated April 03, 2026, document(s) (1) Type A citation(s) which shall be posted for 30 consecutive days as there is(are) immediate risk(s) to the health, safety, or personal rights of children in care.
