On date and time listed, Licensing Program Analysts (LPAs) Perla Ordones and Eric Ramos arrived at the facility to conduct an annual inspection as part of a compliance review. LPAs toured the facility, inside and out, records were reviewed, and the following was observed and/or discussed: Normal days and hours of operation are: Monday \342\200\223 Friday; 06:30AM \342\200\223 05:30PM. OFF-LIMIT AREAS INCLUDE: Entire second floor, living room, kitchen, backyard, and front yard. The facility is operating within the licensed capacity and appropriate ratios. \302\267 Appropriate supervision provided during this inspection. \302\267 A working telephone is present and current number on file. \302\267 Appropriate fire extinguisher, smoke detector and carbon monoxide detector present and were tested by the Licensee during this inspection. \302\267 Fireplace is properly screened to prevent access by children. \302\267 All hazardous items are stored inaccessible to children. \302\267 Toxins are locked. \302\267 Weapons are not present. Licensee understands all guns, weapons and ammunition must be key locked separately and made inaccessible per Title 22 Regulations. \302\267 Stairs are barricaded. \302\267 Verification of control of property on file. \302\267 Facility Sketch, Emergency Disaster Plan & Notification of Parent\342\200\231s Rights poster are posted. \302\267 During inspection, Licensee stated their backyard, front yard, living room, and kitchen areas are off-limits. Licensee stated that the day-care children were last outside October of 2024. Licensee understands that prior to changing the off-limit areas to on-limits they must first notify the department. \302\267 Mandated Reporter Training completed on 01/25/2024. \302\267 Pediatric CPR and First Aid Card completed on 02/03/2024. \302\267 Health & Safety Certificate \342\200\223 completed on 10/14/2005. \302\267 No bodies of water at this time. Licensee understands all bodies of water including ponds, above ground pools & spas, in-ground pools & spas, and some fountains must be properly covered or fenced per Title 22 Regulations. The Department must be notified before and after installation of the above types of bodies of water. In addition, all wading pools or similar product must be emptied immediately after use and stored in an upright position. \302\267 Clean, safe and age appropriate toys. \302\267 Current roster on file. \302\267 Documentation of fire and disaster drills on file \342\200\223 Last drill conducted on 12/02/2024. \302\267 Children\342\200\231s records are not complete. During record review, LPAs observed that C1 and C2 were missing proof of the Family Child Care - Notification Of Parents' Rights (LIC995A). \302\267 Employee\342\200\231s records are not complete. During record review, LPAs observed that S1 and S2 did not have the Mandated Reporter Child Care Providers Training Certificate (AB1207) on file. Additionally, LPAs observed that S1 was missing proof of Tuberculosis clearance. LPAs also observed that S2 was missing proof of the measles vaccine which is a repeat violation that was last cited on 06/06/2024. Due to being a repeat violation in the past 12 months, a civil penalty has been assessed. \302\267 Resident and/or staff records reviewed on 02/06/2025 indicate that all adults who require caregiver background checks have NOT received all required clearances or exemptions. During the inspection, LPAs observed that S1 and S2, who do not currently have eligible fingerprint clearance, were present at the facility and interacting with the day-care children throughout the visit. LPAs observed that S1 and S2 were playing with the day-care children, holding the day-care children, and overall assisting with the day-care children. S1 stated that they have been assisting at the facility since January of 2025 and S2 stated that they have been assisting at the facility since December of 2024. A civil penalty has been assessed. \302\267 The Licensee was informed of their reporting requirements and is provided with the Regional Office\342\200\231s Unusual Incident Reporting email mailbox: UnusualIncidentReportsDO09@dss.ca.gov \302\267 The Licensee can submit transfer forms to associate new individuals or to disassociate someone from your facility at: Associations_Disassociations862@dss.ca.gov LPAs discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPAs also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. LPAs Perla Ordones and Eric Ramos informed licensee Sonia Carias that this report dated 02/06/2025 document(s) one Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPAs Perla Ordones and Eric Ramos informed the licensee Sonia Carias to provide a copy of this licensing report dated 02/06/2025 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. During the exit interview, the LICENSEE Sonia Carias, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.
