On date and time listed, Licensing Program Analysts (LPAs) Perla Ordonez and Eric Ramos arrived at the facility to conduct an annual inspection as part of a compliance review. LPAs toured the facility, inside and out, records were reviewed, and the following was observed and/or discussed: Normal days and hours of operation are: Monday \342\200\223 Friday; 07:00AM \342\200\223 05:00PM. OFF-LIMIT AREAS INCLUDE: Entire second floor, Kitchen, Dog Area, and Garage. The facility is operating within the licensed capacity and appropriate ratios. \302\267 Appropriate supervision provided during this inspection. \302\267 A working telephone is present and current number on file. \302\267 Appropriate smoke detector and carbon monoxide detector present and were tested by the Licensee during this inspection. However, during the inspection, LPAs observed that Licensee did not have the required 2A:10BC fire extinguisher and instead had a 1-A:10B-C fire extinguisher. \302\267 Fireplace is properly screened to prevent access by children. \302\267 All hazardous items are stored inaccessible to children. \302\267 Toxins are locked. \302\267 Weapons are not present per Licensee Sonia Carias. Licensee understands all guns, weapons and ammunition must be key locked separately and made inaccessible per Title 22 Regulations. \302\267 This is a two story home. Stairs are barricaded. \302\267 Verification of control of property on file. \302\267 Facility Sketch, Emergency Disaster Plan & Notification of Parent\342\200\231s Rights poster are posted. \302\267 Mandated Reporter Child Care Providers Training expires on 01/18/2028. \302\267 Pediatric CPR and First Aid Card expires on 02/2026. Licensee attempted to renew their CPR/1st Aid certificate however they completed the training through a vendor who is not EMSA approved and who does not meet regulation requirements. LPAs informed Licensee of regulation requirements and informed Licensee that they must complete a valid pediatric CPR/1st Aid training. \302\267 Health & Safety Certificate \342\200\223 completed on 10/14/2005. \302\267 No bodies of water at this time. Licensee understands all bodies of water including ponds, above ground pools & spas, in-ground pools & spas, and some fountains must be properly covered or fenced per Title 22 Regulations. The Department must be notified before and after installation of the above types of bodies of water. In addition, all wading pools or similar product must be emptied immediately after use and stored in an upright position. \302\267 Clean, safe and age appropriate toys. \302\267 Current roster on file. \302\267 Documentation of fire and disaster drills on file \342\200\223 Last drill conducted on 01/20/2026. \302\267 Children\342\200\231s records are not complete. During record review, LPAs observed that C1 was missing a Acknowledgment of Receipt of Licensing Reports (LIC9224) dated for 02/06/2025 while C2 was missing a LIC9224 dated for 09/11/2025. \302\267 Additionally, LPAs advised Licensee on Operation of a Family Child Care Home regulation. LPAs also advised Licensee on which LIC627 form to utilize for child care. LPAs advised Licensee on Infant Safe Sleep regulations, specifically regarding the times of the 15 minute sleep checks. \302\267 Employee\342\200\231s records are complete. LPA advised Licensee on Personnel Records regulations. Licensee asked for clarification on what immunizations are needed for employees/assistants. LPAs informed Licensee of regulation requirements. \302\267 The Licensee was informed of their reporting requirements and is provided with the Regional Office\342\200\231s Unusual Incident Reporting email mailbox: UnusualIncidentReportsDO09@dss.ca.gov \302\267 Resident and/or staff records reviewed on 02/24/2026 indicate that all adults who require caregiver background checks have received all required clearances or exemptions. \302\267 The Licensee can submit transfer forms to associate new individuals or to disassociate someone from your facility at: Associations_Disassociations862@dss.ca.gov LPAs discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for and removing any recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22- 02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. A notice of site visit was given to licensee and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. During the exit interview, the Licensee Sonia Carias, confirmed that there are no Registered Sex Offenders living in the facility and LPAs completed the RSO profile in FAS.
