On Friday, November 14, 2025 at approximately 09:58AM, Licensing Program Analyst (LPA) Jesse Gardner conducted an unannounced Case Management inspection to follow-up on an Unusual Incident Report (UIR) submitted to Community Care Licensing CCL on November 8, 2025. LPA met with Director Michelle Cortes. LPA interviewed the Executive Director, the Director, and Child One (C1) during this inspection. On November 8, 2025, CCL received information via UIR that on November 7, 2025 a parent reported to staff on November 4, 2025, a parent noticed Staff One (S1) pick up a child (C1) and sternly place them on a chair for a time out further witnessing C1 upset and crying. The parent went on to say that S1 stood over C1 forcing C1 to sit in the chair. The parent felt the action was inappropriate. LPA reviewed footage of the incident on today's date, and camera footage revealed S1 running after and forcefully picking up C1, and briskly walking C1 over to an area where there was said to be a "thinking chair" (per the Director interview). LPA observed S1 was a good distance from the chair where she used momentum to place C1 into the chair. C1's parent was advised of the incident, and no injury was reported to C1. Executive Director Kubel advised, S1 was relieved of their position concluding the incident. Based on information gathered, the CCC did not act appropriately and a violation of Title 22 was identified. An exit interview was held with Executive Director Angie Kubel. A copy of this report was issued, along with copies of the LIC809-D, Appeal Rights, and a Notice of Site visit. This report shall be public record for three years.
