This requirement was not met as evidenced by: Based on the record review and interview, the Licensee did not meet the Operation of a Family Child Care regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. Licensee stated she has not added Child #1 and child #2 to the roster.

Mandated Reporter certificate on file expired on 3-1-2020 and Licensee stated she does not have an updated certificate.
