What the inspector wrote

On March 7, 2024 Licensing Program Analyst (LPA) Elyse Jones and Office Technician Eric Ramos (OT) arrived at the facility to conduct a Case Management-Other. The LPA and OT met with Maria Estrada, Licensee and Maria Garcia, Licensee. Licensees were advised that Eric was present to assist with Spanish translations. During the inspection, the LPA and OT toured the facility inside/outside, census were taken, and documentation was collected. A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with Maria Garcia, Licensee.

    Based on the record review, the Licensee did not meet the above regulation which poses an immediate Health, Safety & Personal Rights risk to the children in care during file review, LPA reviewed the facility criminal record clearances and associations. LPA determined R1 did not have a criminal record clearance.

    Based on the record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care during file review, LPA was unable to review immunizations for C4.

    Based on the record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care during file review, LPA was unable to review TB clearance for S2 and R1.
