What the inspector wrote

It was assumed that the diaper had been soiled for some time due to the sticky consistency of the contents and not due to any rash or redness observed. The licensee and their staff conduct diaper checks throughout the day. Diapers are checked prior to nap time and after. During the visit conducted on 1/27/2026, LPA observed facility staff inform a parent/legal guardian that their child’s diaper was wet. Staff offered to change the diaper and the parent/legal guardian preferred to do it themselves. A child was picked up early from the facility, during nap time. After about an hour, a picture was received by the licensee, of the child, showing swelling above the eye, on the eyebrow, with an inquiry as to what had occurred. Prior to napping the child was observed playing and no complaints of pain or that there had been an incident/injury. The child never cried or complained about anything throughout the day. LPA was not successful after making multiple attempts to obtain additional information from pertinent sources surrounding this alleged injury. Based on the information obtained, and although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegation is unsubstantiated. A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with the Licensee Erica Verdugo.
