Inspector’s narrative

What the inspector wrote

On the date and time listed, Licensing Program Analyst (LPA) Samuel Lopez arrived at the facility to conduct an inspection regarding a separate matter/investigation. In the process of the investigation, information was provided to LPA Lopez that upon enrollment children are verbally informed of the rules and procedures at the facility. This includes disciplinary consequences that include time out by sitting on a chair/bench or having to put their arms up over their head and face the wall. This process was demonstrated to the LPA and information was also obtained that this form of discipline is utilized at the facility. See LIC809-D for cited deficiency. LPA Samuel Lopez informed Licensee Angie Paco that this report dated 4/29/2026 document(s) (1) Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA Samuel Lopez informed the Licensee Angie Paco to provide a copy of this licensing report dated 4/29/2026 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. A notice of site visit was given to Licensee Angie Paco and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with the Licensee Angie Paco

    Personal Rights: To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse, or other actions of a punitive nature, including, but not limited to: interference with eating, sleeping or toileting; or withholding shelter clothing, medication or aids to physical functioning. This requirement is not met as evidenced by information obtained that children are instructed to face the wall and lift their arms over their head as a form of discipline. This poses/posed an immediate personal rights risk to children in care.
