What Happened
Case management visit conducted in response to unusual incident report. Staff worked alone without meeting director qualification requirements, violating CCR 101215.1(f).
Observations
- Staff worked alone in director capacity without meeting personnel qualifications
- Facility did not have qualified staff to cover during director's absence - violation of CCR 101215.1(f)
