APPLICATION FOR INITIAL LICENSE
Based on interview and record review, the licensee did not comply with the section cited above as LPAs observed that S1 did not have proof of Tuberculosis clearance that meets regulation requirements which poses a potential health, safety or personal rights risk to persons in care.

PERSONAL RIGHTS
Based on observation and record review, the licensee did not comply with the section cited above as LPAs observed an infant in a rocker located on top of a changing table. Although S1 was next to the infant, according to page 2 of the user manual found on Amazon.com, it states use rocker "only on the floor, never use on an elevated surface" which posed a potential health, safety or personal rights risk to persons in care.

ARTICLE 2. Administration of Child Day Care Licensing
Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that Licensee and S1 were both missing current Mandated Reporter Child Care Provider certificates (AB1207). Licensee's AB1207 expired on 11/2025 and S1 did not have a AB1207 certificate at all which poses a potential health, safety or personal rights risk to persons in care.

Based on interview and record review, the licensee did not comply with the section cited above as LPAs observed that S1 was missing proof of the pediatric CPR/1st Aid certificate and LPAs observed that S1 was left alone with day-care children during the inspection which posed a potential health, safety or personal rights risk to persons in care.
