What the inspector wrote

On date and time listed above, Licensing Program Analysts (LPAs), Susan Brewer and Chase Atherton, arrived at the facility to conduct annual inspection. LPAs were granted entry by Facility Representative Debbie Andrews Office Manager. The LPAs toured the facility inside and out, reviewed records, took a census and observed and/or discussed the following. The Director Katherine Pasley, arrived at 8:30 AM. Days and hours of operation are Monday - Friday; 6:30 AM - 6:30 PM; Preschool is operated in the Orange, Yellow, Purple and Red rooms. The facility has waivers on file. The inspection consisted of reviews of the CARE tool domains. The inspection found the facility to be in compliance. If updates are made, the facility representative will submit the following documents to Licensing within 30 days: 1. LIC 500 Personnel Report 2. LIC 610 Emergency & Disaster Plan 3. Parent Handbook/ Program Curriculum/Admission policies and procedures/ fee schedule 4. LIC 309 Administrative Organization and 5. LIC 308 Designation of Administrative Responsibility The following items have been posted and are current: License; Emergency Disaster Plan (LIC610); Facility sketch; Earthquake Preparedness Checklist (LIC9148; Parent\342\200\231s Rights Poster (PUB393); Personal Rights (LIC613A); Child Car Seat Law. The agency does not provide meals, therefore Menus are not posted for the public. The facility is operating within the scope of license with appropriate ratios and supervision during this inspection. The LPAs took a census of 48 children supervised by 4 staff. Medications are stored inaccessible to children. The LPAs observed the medication stored in a locked cabinet. The licensee was in compliance with IMS records. A fully charged fire extinguisher (2A:10BC) was observed and tagged by the fire department and needle in the green. A smoke detector and carbon monoxide detector were present and tested by the licensee during this inspection on 12/10/2025. Hazardous and dangerous items are stored inaccessible to children. Poisons and toxins were observed in an upper cabinet with latches, however the 4 items observed were not locked. The licensee removed the items in the presence of the LPAs and stored the items in a locked area. Classrooms and floors are clean, free of hazards and rooms are equipped with age-appropriate furniture. Bathrooms were observed to be safe, sanitary and in operating condition. Per facility representative, Katie Pasley stated there are no weapons are stored at the facility. There are no accessible bodies of water present. The licensee Katie Pasley, understands that all wading pools or similar products must be emptied immediately after use and stored in an upright position. Outdoor activity areas are enclosed by appropriate fences, free of hazards and have age and size equipment in good condition. Areas around or under high climbing equipment have rubber matting and artificial turf materials to absorb a fall. The facility has a kitchen however they do not food preparation or store food for children at the facility. The facility does not post menus. The facility does not provide meals or snacks for children. The children bring food from home. Measures are taken to keep the facility free of pests and rodents. Storage containers for solid waste have tight-fitting covers in good repair. Uncontaminated drinking water is readily available both indoors and outdoors: Children bring water bottles from home, and the agency refills the water bottles as needed with purchased water. Sign in/Sign out record was reviewed and meets regulation requirements. A Staff member is present with CPR/First Aid which expires on 08/2027. Opening and closing staff member\342\200\231s CPR/First Aid expires on 08/2027. Mandated Reporter General Training: 01/24/2024; AB1207 training certificate expires 01/24/2026 Director completed Health and Safety Training on 06/30/2020 Children files are complete on 12/10/2025. Staff have received on-the-job training for housekeeping, sanitation and universal health precautions. Staff files are complete: 12/10/2025 Fire & earthquake drills are conducted/documented every six months. Last fire drill on 10/16/2025 at 11:15 AM with 55 students present. Facility Representative was informed that the Department has inspection authority per Health and Safety Codes. Facility representative wa s reminded of the following: 1. Facility staff are responsible to know the regulations for providing care 4. Failure to meet the posting requirements shall result in an immediate civil penalty. 5. To receive important licensed -related information to licensed facilities, visit the CCLD Important Information website at https://www.cdss.ca.gov/inforesources/community-care-licensing/subscribe and select the Child Care option to receive email communication. - AB 1207 \342\200\223 Mandated Child Abuse Reporting: Child Day Care Personnel Training, beginning January 1, 2018 \342\200\223 Requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. AB2960 \342\200\223 Childcare and development services, online portal \342\200\223 Effective June 20, 2022 - This bill requires the State Superintendent of Public Instruction (SSPI), within the California Department of Education (CDE), to develop and post on CDE's website a comprehensive childcare and development services online portal for families and providers by June 30, 2022. Licensee Katie Pasley was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California. Licensee Katie Pasley was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. The agency building is new and construction completed in September 2012. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test as specified in Health and Safety Code section 1597.16. LPA referred licensee Katie Pasley to the Department website for lead: https://www.cdss.ca.gov/inforesources/child-care-licensing/water-testing-information , to access the attestation form and submit a written statement of the exemption to the department. This facility provides Incidental Medical Services \342\200\223 IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children\342\200\231s, personnel, and administrative records. For IMS information see PIN 22-02-CCP. During the exit interview, the licensee Katie Pasley, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. The licensee Katie Pasley can submit transfer forms to associat e new individuals or to disassociate someone from your facility at: Associations_Disassociations862@dss.ca.gov .
