What the inspector wrote

A case management visit is being conducted in response to the receipt of an unusual incident report (UIR) from the facility. However, during an interview with the facility representative Anita Jones, Licensing Program Analyst (LPA) found that a visit was already conducted to follow up for this UIR on 5/5/2025. Therefore, the current inspection is being ended without further investigation, as the UIR has already been addressed in full in a report dated 5/5/2025. An exit interview was conducted and a copy of this report was provided to facility representative.
