Inspector’s narrative

What the inspector wrote

On the date and time listed, Licensing Program Analyst (LPA) Perla Ordonez arrived at the facility in response to information received by the Department on 04/01/2026. LPA was granted entrance into the facility by Licensee Susana Perez. LPA explained the purpose of the visit and conducted a facility tour to take census. The information sent to the Department noted that an incident occurred on 03/26/2026 involving an individual residing in the home and law enforcement. The licensee confirmed that an incident occurred around that time, during day-care hours, and Licensee acknowledged that they did not report the incident to Community Care Licensing (CCL). According to interviews conducted, no day-care children were physically injured during this incident. Based on the information gathered, due to the Licensee not reporting the incident to CCL, there was a violation of Title 22 regulation 102416.2 – Reporting Requirements. See LIC809-D for cited deficiency. The Licensee was informed of reporting requirements and is provided with the Regional Office’s Unusual Incident Reporting email mailbox: UnusualIncidentReportsDO09@dss.ca.gov A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with the Licensee Susana Perez.
