What the inspector wrote

On August 22,2024 at 12:15 PM Licensing Program Analyst (LPA) Courtnee Peebles unannounced at CHILDRENS LIGHTHOUSE LEARNING CENTER (CCC) to conduct a case management visit. On 08/22/2024 CCLD received an unusual incident report stating Child 1 (C1) escaped classroom #3 and was found outside on the playground. Facility records were reviewed, and Staff 1 was interviewed. LPA toured the facility to observeD the area in which the incident occurred and based on information gathered, the facility did violate Title 22 regulations 101229(a)(1) Lack of supervision. Based on interviews,S1 was grabbing a book to read to the children during circle time, and by the time S1 turned back around they felt like something was missing. S1 stated, they then began to count the children in care when they realized C1 was missing. S1 stated, they then ran to AD and the AD stated, C1 was outside in the back on the playground. S1 stated, the cook was in the back doing the food order when they noticed little legs and called out for S1, however S1 was not present. S1 stated, this entire incident could not have been more than five minutes. The facility is being cited for Title 22 Regulation Section 101229 Responsibility for Providing Care and Supervision (a) The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. An exit interview was conducted, and a copy of this report was provided to owner, Rachel Choi A notice of site visit was also provided and must remain posted for 30 days.
