What the inspector wrote

On September 26 ,2025 at 11:00 AM Licensing Program Analyst’s (LPA’S) Courtnee Peebles arrived unannounced at CHILDRENS LIGHTHOUSE LEARNING CENTER (CCC) to conduct a case management visit. On 08/20/2025 CCLD received an unusual incident report stating Child 1 (C1) sustained a fractured pinky finger. Facility records were reviewed, and Director (D) was interviewed. LPA toured the facility to observe the area in which the incident occurred and based on information gathered, the facility acted appropriately, and no violations have been identified. Based on interviews, C1 was playing on the mat when C2 walked passed C1 stepping on C1's finger. C1 immediately pulled their pinky finger from under C2's shoe causing swelling. C2 was immediately taken to administration where TLC and an ice pack was provided. C1's guardian was contacted informing them of the incident. C1's guardian informed the childcare facility they were taking C1 to the hospital to get their finger evaluated. The next day C1 returned with a splint on their finger. The day after C1 returned with their fingers taped together. That following Monday C1's guardian informed the department that C1 was taken to the hospital once more where doctors informed them C1's finger was healed and there was no further action required. In addition, the facility’s staff reported the incident timely to the Department. An exit interview was conducted and copy of this report was provided to Director, Michelle Rubalcaba. A notice of site visit was given and must remain posted for 30 days.
