What the inspector wrote

On 7/10/2024 at 12pm, Licensing Program Analyst (LPA) Jeanette Sanchez conducted a case management visit to address an issue which arose during a complaint investigation (Control number 10-CC-20240411151306 ) conducted. LPA met with licensee Merery Ledezma. During the investigation, an interview disclosed that the licensee leaves infants in high chairs for extended periods of time. It was further elaborated to say that the infants remain in the high chairs when they are not eating or playing and that they were told it was for the protection of the infants. Other interviews corroborated the statement. While conducting the initial investigation on 4/16/2024, LPA noted two infants in high chairs who were neither eating or playing. The length of time before the children were taken out of the high chairs was approximately 10 minutes. On 6/12/2024, an infant was in a bouncy seat while licensee cleaned the kitchen. LPA consulted about infant being in an off limit area as well as being in the bouncy seat. During today's inspection, infant was once again in the off limit area in the bouncy seat. Also during the inspection, it was found that two children have not been added to the roster. This deficiency was cited on 4/16/2024. Based on interviews and observation, a deficiency has been cited. See LIC809-D for cited deficiencies. An exit interview was conducted, and this report was reviewed with the licensee Merery Ledezma. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

Infant was observed in an infant device not permitted for use in daycares. This poses a potential health, safety or personal rights risk to persons in care.
