Inspector’s narrative

What the inspector wrote

On date and time listed above, Licensing Program Analyst (LPA), Susan Brewer arrived at the facility to conduct annual inspection. LPA was granted entry by Facility Representative Gloria Martinez. LPA toured the facility inside and out, reviewed records, took a census and observed and/or discussed the following. The Director Teresa Garcia arrived at 8:40 AM. Program Director Art Cabrera arrived at 11:30 AM. Days and hours of operation are Monday-Friday, 7:00 AM 5:00 PM. The inspection consisted of reviews of the CARE tool domains. The inspection found the facility to be in compliance except as noted on the LIC809D. If updates are made, the facility representative will submit the following documents to Licensing within 30 days, on or before 06/06/2026: 1. LIC 500 Personnel Report – Update requested. 2. LIC 610 Emergency & Disaster Plan- Received during inspection. 3. Parent Handbook/ Program Curriculum/Admission policies and procedures/ fee schedule 4. LIC 309 Administrative Organization 5. LIC 308 Designation of Administrative Responsibility 6. LIC200A Application update received during inspection. The following items have been posted and are current: License; Emergency Disaster Plan (LIC610); Facility sketch; Earthquake Preparedness Checklist (LIC9148; Parent’s Rights Poster (PUB393); Personal Rights (LIC613A); Child Car Seat Law, and Menu. The facility is operating within the scope of license with appropriate ratios and supervision during this inspection. Medications are stored inaccessible to infants in a locked box located in the classroom. A fully charged fire extinguisher (2A:10BC) was observed and tagged by the fire department on 11/04/2025 and needle in the green. A smoke detector and carbon monoxide detector were present and tested by the licensee during this inspection on 05/06/2026. Hazardous and dangerous items are stored inaccessible to infants. Poisons and toxins are locked. Classrooms and floors are clean, free of hazards and rooms are equipped with age-appropriate furniture. No restrooms in this classroom, only changing table with sink, observed to be safe, sanitary and in operating condition. Licensee representative stated there are no weapons are stored at the facility. There are no accessible bodies of water present. All wading pools or similar product must be emptied immediately after use and stored in an upright position. Outdoor activity areas are enclosed by appropriate fences, free of hazards and have age and size equipment in good condition. Areas around or under high climbing equipment have material that absorbs a fall: Artificial turf. Food preparation area is clean, free of litter, and vermin. Food is stored appropriately and protected from contamination. Menus are current, posted and visible for authorized representatives and available upon request. Measures are taken to keep the facility free of pests and rodents. Storage containers for solid waste have tight-fitting covers in good repair. Uncontaminated drinking water is readily available both indoors and outdoors. Sign in/Sign out record was reviewed and meets regulation requirements. A Staff member is present with CPR/First Aid which expires on 02/21/2028. Opening and closing staff member’s CPR/First Aid expires on 10/18/2027. Director completed Health and Safety Training 08/08/2025 Mandated reporter General: 07/09/2025; AB 1207 Child Care Expires: 07/2027 Infant files are NOT complete on today’s date. Missing documentation of 15-minute checks with date, name, time of checks and staff initials for May 1-5, 2026. Individual Infant needs and service plans are complete and current for each infant. Infants up to 12 months have a sleeping plan LIC9227 on file. Staff have received on-the-job training for housekeeping, sanitation and universal health precautions. Staff files are complete on today’s date. Fire & earthquake drills are conducted/documented every six months. Last fire drill at 9:40 AM on 04/29/2026, 100 children present and 28 staff. Facility Representative was informed the Department has inspection authority per Health and Safety Codes Facility representative was reminded of the following: 1. Unusual Incident Reporting email: UnusualIncidentReportsDO09@dss.ca.gov 2. Access to forms & Regulations online at www.ccld.ca.gov 3. Facility staff are responsible to know the regulations for providing care 4. Failure to meet the posting requirements shall result in an immediate civil penalty. To receive important licensed -related information to licensed facilities, visit the CCLD Important Information website at https://www.cdss.ca.gov/inforesources/community-care-licensing/subscribe and select the Child Care option to receive email communication. - AB 1207 – Mandated Child Abuse Reporting: Child Day Care Personnel Training, beginning January 1, 2018 – Requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. AB2960 – Childcare and development services, online portal – Effective June 20, 2022 - This bill requires the State Superintendent of Public Instruction (SSPI), within the California Department of Education (CDE), to develop and post on CDE's website a comprehensive childcare and development services online portal for families and providers by June 30, 2022. Director Teresa Garcia was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California. Director Teresa Garcia was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. The Licensee can submit transfer forms to associate new individuals or to disassociate someone from your facility at: Associations_Disassociations862@dss.ca.gov . CCC COMPLETED TESTING COMPLETED ON 11/23/2022 AND NO LEAD EXCEEDANCES Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test as specified in Health and Safety Code section 1597.16. For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP.). LPA verified that the lead testing sample was completed 10/23/2022 in accordance to the Written Directives outlined in PIN 21-21.1-CCP. Reported results received 11/23/2023, no action level exceedance. LPA referred Director Teresa Garcia to the Department website for lead: Lead Toxicity Prevention and Water Testing Information ( https://www.cdss.ca.gov/inforesources/child-care-licensing/water-testing-information . This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. During the exit interview, the Director Teresa Garcia, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. LPA discussed the safe sleep regulations with Director Teresa Garcia and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also Director Teresa Garcia of the importance of checking for and removing any recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Deficiency was issued on today’s date for a type B violation. No civil penalties were issued on today’s date. A notice of site visit was given and must remain posted for 30 days. The LPA observed the licensee post the notice for public view prior to exiting the facility. Exit interview conducted and report was reviewed with the licensee Director Teresa Garcia.
