On April 29, 2025 at approximately 1:00 PM, Licensing Program Analyst (LPA) Tricia Danielson arrived unannounced to the facility to conduct a case management visit in response to the receipt of an unusual incident report (UIR) from the facility. The UIR was received by the Department on April 23, 2025 describing and incident between Child #1(C) and Child #2(C). During today's visit, LPA interviewed (3) staff but did not interview C1 or C2 due to nap schedules. Records for C1 and C2 were reviewed as well. Based on information gathered, facility staff acted appropriately upon the discovery of the incident and provided required notifications to pertinent parties. No violations have been identified. An exit interview was conducted and a copy of this report was provided to facility staff along with a Notice of Site Visit and LIC811- Confidential Names list.
