CONTINUED FROM LIC 9099: Based on interviews and record review, the allegations that licensee is not providing adequate supervision resulting in altercation between children in care may have occurred, however, there is not a preponderance of evidence to support or dismiss the allegations. Therefore, the allegations are unsubstantiated at this time. A copy of this report as well as a copy of the Appeal Rights were provided to Director, Desi Andrews on this date and time. A Notice of Site Visit was posted and must remain posted for 30 days for public review. CONTINUED FROM LIC 9099: Based upon LPAs observations and information gathered through record reviews, the preponderance of evidence standard has been met, therefore, the above allegation is found to be SUBSTANTIATED. The facility is being cited in accordance with California Code of Regulations, Title 22, Division 12, Chapter 1, Section 101218.1(a)(2)(B): Admission Procedures and Parental and authorized Representative's Rights on the attached LIC 9099D. A copy of this report as well as a copy of the Appeal Rights were provided to Director, Desi Andrews on this date and time.
