This is an amended report to a visit conducted on 7/7/25. On 7/18/25 at 1:10 PM, Licensing Program Analyst (LPA) Sandra Pulido made an unannounced visit for a Case Management inspection to deliver an amended report for an annual conducted on 7/7/25. LPA met with Director Desiree Andrews to correct errors in the report pertaining to the citation given in error regarding the facility not having staff on site with current Pediatric First Aid certification. LPA toured the facility and census was taken. An exit interview was conducted, signatures were obtained for the amended pages, and a copy of this report was provided to Director Desiree Andrews. A Notice of Site Visit was also provided, and facility representative was reminded the notice must be posted for 30 consecutive days.
