What the inspector wrote

Licensing Program Analysts (LPAs) Ana Noble and William Chancellor arrived at the facility on a case management inspection to follow-up on an Unusual Incident Report (UIR) which occurred on March 6, 2023. At the time of visit, LPA toured the facility, took census, interviewed relevant individuals and met with Jane Osuna, Director to discuss the purpose of the visit. The reported incident took place on March 6, 2023 and was witnessed by Staff 1-2 and Director. The incident that was reported was of Child 1 who got their two right fingers caught between two nesting blocks while playing. During todays inspection of the stacking blocks/toy, indicated that the toy was age appropriate for use with by Child 1. The age indicated was 18 months to 4 years of age. At approximately 12:30 pm Staff 1 observed Child 1 was playing with the stacking blocks, when child told Staff 1 that they need help. Staff 1 went to assist Child 1 and noticed that their fingers were stuck and as Staff 1 attempted to remove the block Child 1 started to cry. Staff 1, then requested assistance from Staff 2, who immediately started to attempted to remove the child fingers however, Child 1 would start to cry. At this time Staff contacted the Director due to the child's fingers starting to swell up and turn purple. Director then immediately saw the child fingers and left to call emergency service and the child's parents. The fire department arrived approximately 10-15 minutes after call. The firemen then assessed the situation and was able to dislodge, Child 1 fingers from the stacking blocks without further incident. Child was not taken in for further treatment by the emergency services. The child parent took child home and Child 1 required no further medical attention or treatment. Child returned to school their next school date. After review of facility documentation and interviews conducted, there is not a violation of Title 22 Regulations pertaining to this unusual incident reported to the department on March 6, 2023. Due to this incident the stacking blocks have been removed and discarded. No cited deficiencies during this inspection . An exit interview was conducted with Jane Osuna, Director. Appeal rights discussed and a copy of this report was provided.
