Inspector’s narrative

What the inspector wrote

On the date and time listed, Licensing Program Analysts (LPAs) Perla Ordones and Raymond Moorehead arrived at the facility for the purposes of concluding a Case Management inspection which was initially conducted on 02/27/2025 in response to the receipt of an Unusual Incident Report (UIR) that was submitted by the facility on 02/13/2025. LPAs were granted entry and explained the purpose of today's inspection, completed a facility tour, and obtained a census. LPAs made observations, reviewed pertinent documentation, and conducted interviews with pertinent parties related to the self reported incident. It was reported that on 02/12/2025 a day-care child was left unattended in a classroom during transition time. LPAs reviewed pertinent documentation such as the "Monty's Montessori Academy of Calimesa - Event Report" which stated that a day-care child "was left unattended in the classroom on accident while the class transitioned from the classroom to the auditorium". Additionally on the "Monty's Montessori Academy of Calimesa - Event Report" the event was labeled "Accidentally left behind". LPAs interviewed pertinent parties who stated that upon conducting a "name-to-face check" once they had transitioned the staff noticed that the child was unaccounted for. It was stated that the child had gone back inside the classroom after lining up to transition in order to obtain their water bottle. Pertinent parties stated that at same the time the child went back inside for their water bottle, other children were being dropped off, and the staff closed the classroom door. Pertinent parties stated that the child was later found alone in the Mountain Classroom by the window. It was stated that the child was "waiting" for someone to come back for them and that the event made the child feel sad. The day-care child's authorized representative(s) were informed of the incident. Per regulation 101229(a)(1), Responsibility for Providing Care and Supervision, "No child(ren) shall be left without the supervision of a teacher at any time...Supervision shall include visual observation". Based on information obtained during the investigation, it has been determined that this incident is a case of absence of supervision and is a violation of Title 22 regulations. See LIC809-D for cited deficiency. A Civil Penalty has been assessed during this inspection. Payment is due when billed and the check(s) or money orders shall be made payable to the “California Department of Social Services”. YOU WILL RECEIVE AN INVOICE IN THE MAIL. DO NOT SEND MONEY UNTIL YOU RECEIVE YOUR INVOICE. DO NOT SEND CASH. LPAs Perla Ordones and Raymond Moorehead informed the Assistant Director Jennifer McClintock that this report dated 03/19/2025 document(s) one Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPAs Perla Ordones and Raymond Moorehead informed the Assistant Director Jennifer McClintock to provide a copy of this licensing report dated 03/19/2025 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Please note that LPAs left the facility at 11:55AM and returned at 02:12PM. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with Assistant Director Jennifer McClintock.

    (2) To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs.This requirement is not met as evidenced by: Based on interview and record review, it was determined that a child was kept in the isolation bathroom rather than the main isolation area for approximately 1.5 hours despite the child's symptoms not needing restroom amenities to treat which poses a potential personal rights risk to children.

  • RESPONSIBILITY FOR PROVIDING CARE AND SUPERVISION

    (1) No child(ren) shall be left without the supervision of a teacher at any time... Supervision shall include visual observation.This requirement is not met as evidenced by: Based on interview and record review, the licensee did not comply with the section cited above as it was reported that a day-care child was left alone in a classroom during transition time which posed an immediate health, safety or personal rights risk to persons in care.
