On the date and time listed, Licensing Program Analyst (LPA) Perla Ordonez arrived at the facility in response to the receipt of an Unusual Incident Report (UIR) that was submitted by the facility. The UIR was received by the licensing agency on 04/29/2026. Please note that LPA attempted to conduct the visit on the 05/08/2026 but the facility was closed that day. LPA was granted entrance into the facility by Licensee Dolores Gambino. LPA explained the purpose of the visit and conducted a facility tour to take census. The UIR documents an incident wherein a day-care child received a head injury while playing outside. LPA observed a video where the day-care child was outside running before tripping over a rug that was folded over itself and hitting their head. Licensee stated that they had planned to throw the rug away and so folded it and placed it in that area to be thrown away later. Based on the video observed the Licensee did not provide a safe play environment for the children in care. Based on the observations made and information gathered, there was a violation of section 102423(a)(2). A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with the Licensee Dolores Gambino.
