On 12/03/2025, Licensing Program Analyst (LPA) Jesse Gardner conducted an unannounced Case Management visit to the facility to follow up on deficiencies that were cited on 04/18/2025. Present when LPA arrived was the Licensee and their Assistant's Harmony DeWolf, Micah DeWolf, and Joshua DeWolf. During the initial visit on 04/18/2025, LPA toured the outside of the facility and found a pool with a lock that was not to 60". At the time, the Licensee agreed to correct the fence, and move the gate lock up to 60 inches. Additionally, the gap at the bottom of the fence can not exceed 2", and on the initial visit, the gap exceeded the requirement. The Licensee agreed to correct the deficiencies and provide proof of such to LPA by 05/16/2025. On today's visit, LPA found the following: 1. On today's date, LPA noted all areas to be in compliance not to exceed 2". 2. The lock to the gate on the fencing meets the standard at 60". Deficiencies cleared. An exit interview was conducted with Licensee April Muniz-Dewolf, and a copy of this report was reviewed with and provided along with a copy of the Appeal Rights. A Notice of Site Visit was also provided and must be posted for 30 days.
