What the inspector wrote

On February 20, 2025, at 1:30 pm, Licensing Program Analyst (LPA) Courtnee Peebles arrived unannounced to the facility to conduct a case management visit due to an unusual incident report received for an incident that occurred on 2/07/2025, reporting child (C1) was left unsupervised while transitioning from outside to inside. LPA met with Director Patricia Morley and conducted a tour and census of the facility. During the visit, LPA interviewed Director and Staff 1(S1). The interviews revealed the class was transitioning from outside to inside when S1 began doing names to faces. S1 was then distracted by another child climbing on the cubbies for the children in care when S1 went to retrieve the child climbing on the cubbies and let the door close behind them. Once the child was removed from the cubbies S1 grabbed their names to faces checklist and counted the children in care and noticed C1 was not in the classroom. S1 shouted C1’s name alarming other staff in the classroom, and both S1 and another staff ran towards the glass door where C1 was visible and waiting patiently. Furthermore, interviews also disclosed C1 was left unsupervised outside for less than one minute. Staff immediately notified C1's parent and in response to the incident, the Director provided supervision training to S1 on policies including the scanning and counting of children. LPA determined that the facility is not in violation of Title 22 Regulation Section 101229 (a)(1). An exit interview was conducted, a copy of this report, and appeal rights were reviewed with and provided to Director Patricia Morley. A notice of site visit will be provided and must remain posted for 30 days.
