What Happened
Complaint investigation substantiated regarding inadequate supervision. Child sustained supracondylar fracture of left elbow when licensee was seated on couch out of visual range of play area. Licensee failed to immediately notify parent of incident. Incident occurred 8/13/24; investigation conducted 2/7/25.
Observations
- inadequate supervision—child injured while licensee seated on couch out of sight of play equipment
- couch positioned in adjacent living room with no direct line of sight to slide in play area
- licensee admitted being seated on couch when child screamed
- child sustained supracondylar fracture of left elbow
- licensee failed to inform parent of incident immediately; instead told parent child was 'coming down with a bug'
- parent discovered injury after child began screaming when arm was touched
- child taken to hospital for medical evaluation and diagnosed with supracondylar fracture
