What the inspector wrote

The facility provided LPA with 13 months of service receipts, dated from 05/28/2022 to 04/03/2025, confirming regular pest control visits. Site Director Yulisa Medina also reported that a professional cleaning company services the facility nightly to maintain cleanliness and sanitation. Additionally, LPA conducted a physical inspection of the facility and did not observe any evidence of a pest infestation. Based on the information obtained through interviews, documentation, and facility inspection, there is insufficient evidence to support the allegation. Based on the information obtained through interviews and documentation provided, the allegation could not be verified. There is insufficient evidence to substantiate that staff failed to maintain the facility free of pests. Although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the allegations did or did not occur, therefore the allegations are UNSUBSTANTIATED. An exit interview was conducted and a copy of this report along with the appeal rights were provided to Site Supervisor Yulisa Medina. A notice of site visit was provided and must remain posted for 30 consecutive days.
