What Happened
3-year required inspection found multiple infant safe sleep violations including prohibited sleep sacks and missing 15-minute sleep check logs, plus debris in outdoor play area and incomplete emergency disaster plan.
Observations
- Child (age 1) sleeping on crescent-shaped nursery pillow in play yard (facility mistakenly believed child was 2 years old)
- Two prohibited sleep sacks observed in facility
- Missing 15-minute sleep check logs for children C1, C2, C3
- Debris observed in on-limits backyard
- Missing second relocation site on emergency disaster plan (LIC610A)
- CPR and First Aid Card expired 07/2023
