Follow-up on corrections·October 29, 2024

On the above date and time Licensing Program Analysts (LPAs) Susan Brewer and Patricia Berry, arrived at the facility for a separate matter of a complaint investigation. LPAs were greeted and granted entry into the facility by Lead Teacher Denese Keeler. The Lead Teacher Denese Keeler, stated the Director Karen Brazzill and Owner Hanbo Liang were away from the facility. The LPAs took a census of 7 children in care. D. Keeler, stated the LPA Susan Brewer and LPA Aman Lama (not present) are no longer welcome to the facility and need to leave the facility, however LPA Patricia Berry is allowed to remain at the facility. LPAs informed D. Keeler of the inspection authority, Title 22 regulation section 101200(a)(1). The Lead Teacher D. Keeler, stated the Director Karen Brazzill and Owner Hanbo Liang, were on speaker phone to ensure the LPA S. Brewer was escorted out of the facility. The lead teacher D. Keeler stated the owners will accept the civil penalty and knew their rights. LPAs Susan Brewer and Patricia Berry, informed Lead Teacher Denese Keeler that this report dated 10/29/2024 documents a Type A citation, under Title 22 regulation, section 101200(a)(1) Inspection Authority of the department which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care. Also, LPAs Susan Brewer and Patricia Berry informed the D. Keeler to provide a copy of this licensing report dated 10/29/2024 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. See LIC809C Page 2 IF a Civil Penalty has been assessed during this inspection. Payment is due when billed and the check(s) or money orders shall be made payable to the “California Department of Social Services”. YOU WILL RECEIVE AN INVOICE IN THE MAIL. DO NOT SEND MONEY UNTIL YOU RECEIVE YOUR INVOICE. DO NOT SEND CASH. A Type A violation was issued for Title 22 regulation, section 101200 (a)(1) Inspection Authority of the Department, An immediate civil penalty was issued in the amount of $500.00. At 10:22 AM Director K. Brazzill arrived at the facility and concluded the visit. Exit interview conducted and report was reviewed with Lead Teacher Denese Keeler and Director Karen Brazzill. A Notice of Site Visit was given and must remain posted for 30 days. The LPAs verified the Notice of Site Visit was posted prior to exiting the facility.
