Inspector’s narrative

What the inspector wrote

On this date and time, Licensing Program Analysts (LPAs) Laura Mejorado and Tiffanie Diep arrived at the facility to conduct an inspection regarding a separate matter. LPAs met with Director Jennifer House. LPAs toured the facility, took census, and verified associations. During a prior visit to the facility on 09/03/2025, LPA Mejorado observed a staff member (S1) on two occasions alone in the classroom supervision the school age children. On one occasion S1 was alone supervising the children while the lead teacher was out picking up children from school and on the second occasion S1 was alone supervising the children while the lead teacher was on break. When reviewing S1s file it was determined that S1 did not have the credentials to be working alone with the school age children. Unfortunately, the violation was unable to be addressed on 09/03/2025 due to time constraints and the facility closing for the day. Therefore, based on LPA Mejorados observations on 09/03/2025 the facility was found to be in violation of the following Title 22 Regulation: 101516.2 School-Age Child Care Center Teacher Qualifications and Duties (b) As an alternative educational prerequisite, a school-age child care teacher may, pursuant to Health and Safety Code Section 1597.21, substitute 20 training hours for each of the required units of education in Section 101216.1. Units and training hours may be combined to meet the total educational requirement (12 units or 240 training hours, or any combination thereof). See LIC809-D for cited deficiencies. A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with Director Jennifer House.
