REVOCATION OR SUSPENSION OF A LICENSE OR REGISTRATION
Based on record review, the licensee did not comply with the section cited above. The licensee relocated from one facility to another and in doing so, submitted an altered document to meet the requirements for relocation and licensure. The licensee altered the date of completion from 4/28/07 to read 4/28/20. This poses an immediate health, safety or personal rights risk to persons in care.
