What the inspector wrote

On 12/11/2024 at 08:48 AM, Licensing Program Analyst (LPA) Jesse Gardner conducted an unannounced case management visit in regard to an unusual incident report received on 11/21/2024 from Site Director Allison Mineweaser (S1). LPA conducted a tour of the facility and area of incident, interviewed three staff, and made observations. The following was noted. Per incident report, On 11/19/2024, Child 1 (C1) sustained an injury to their head that was later found to be a moderate/severe concussion. The injury occurred while C1 was playing on the playground and went to go down the slide. When C1 went to do this, they bumped their head on the top of the opening to the entrance of the slide. About 45 minutes later, C1 threw up, and concluding parent notification and retrieval, C1 was found to have a moderate/severe concussion after a visit to the hospital. Staff Interviews revealed 2 teachers were outside with 16 children. Interviews further revealed that staff were playing with children and watching them play tag when C1 ran up the stairs to the area of the slide when C1 was injured. Staff retrieved an ice pack, and notified management. LPA conducted video review of the incident, and found statements made to LPA were consistent, and adequate supervision was being provided. Upon observation to the playground and area of incident, LPA found no unusual cuts, burrs, or other protruding object that would abnormally cause an injury to a child. The part of the slide that was stated that C1 hit their head on, is a hard plastic material surrounded on each side by two metal handle bars. Based on all the information obtained, there did not appear to be any violations of Title 22 Regulations pertaining to the reported incident. An exit interview was conducted and a copy of this report and appeal rights were provided at the time visit. A notice of site visit was given and shall remain posted for 30 days.
