What the inspector wrote

Interview with C1 indicated the fall was an accident, and hurt their neck when they fell, and no one else hurt them at any time. LPA reviewed footage of the incident, and found 3 staff (1 teacher, and 2 aides) were with C1, and 15 other children at time of incident. The 3 staff were observed around the play structure and immediately responded to C1 concluding the fall. Staff interview relayed that C1 was provided an ice pack, and parents were notified. Based on the information obtained from interviews, LPA observation, and record review, the allegation was found to be Unsubstantiated. A finding of Unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur. An exit interview was conducted, and a copy of this report was provided along with a copy of the Appeal Rights was provided. A Notice of Site visit was given, and the Licensee understands that it must remain posted for 30 days.
