What the inspector wrote

On Friday, March 13, 2026 Licensing Program Analyst (LPA) Jesse Gardner conducted an unannounced Case Management visit in reference to information that was revealed during complaint number 10-CC-20260306164036. During the investigation in the complaint, it was revealed that on March 5, 2026, a child was seen by a medical center after falling and sustaining a bruise on the playground. The facility did not report the incident to Community Care Licensing neither by phone nor Unusual Incident Report. Based on information gathered, a violation of Title 22 was identified and a citation was issued. An exit interview was held with Director Allison Mineweaser and a copy of this report was issued, along with copies of the LIC809-D, and a Notice of Site visit. This report shall be public record for three years.
