What the inspector wrote

It was alleged that a facility staff did not adequately supervise an infant on changing table. The LPA conducted a review of children\342\200\231s records which indicated that on 10/23/2023 a subject infant/toddler fell from a changing table when a subject staff stepped away from the table. Interviews conducted with pertinent parties confirmed the incident took place. It was also reported there were no injuries to the child as a result of the fall and the child did not require medical attention following the incident. Records obtained during the investigation indicate the licensee documented the lack of supervision on internal form for accidents/illness/injury/report, to provide to the subject child\342\200\231s authorized representatives and kept the record as required, however the facility did not report the incident to the licensing department as required. In addition, the subject staff involved in the incident is no longer employed at the facility. Therefore, the allegation that a facility staff did not adequately supervise infant on changing table, is determined to be substantiated. Based on LPA\342\200\231s observations, interviews, record reviews and by the licensee director's own admission, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. California Code of Regulations, Title 22,101429(a)(1) Responsibility for Providing Care and Supervision for Infants, are being cited on the attached LIC9099D. LPA Susan Brewer informed licensee director Christi Padilla, that this report dated 06/16/2025 documents a Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA Susan Brewer informed the licensee director Christi Padilla to provide a copy of this licensing report dated 06/16/2025 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. A deficiency for a Type A violation was issued on today\342\200\231s date. No civil penalty was issued. Exit interview was conducted and a copy of this report was provided to the Director Christi Padilla. A notice of site visit was provided and must remain posted for 30 days.
