On date and time listed, Licensing Program Analyst (LPA) Perla Ordones arrived at the facility to conduct an annual inspection as part of a compliance review. LPA toured the facility, inside and out, records were reviewed, and the following was observed and/or discussed: Normal days and hours of operation are: Sunday \342\200\223 Monday; 23 hours a day. OFF-LIMIT AREAS INCLUDE: Bedroom 1, bedroom 2, bedroom 3, laundry room, courtyard, and garage. The facility is operating within the licensed capacity and appropriate ratios. \302\267 Appropriate supervision provided during this inspection. \302\267 A working telephone is present and current number on file. \302\267 Appropriate fire extinguisher, smoke detector and carbon monoxide detector present and were tested by the Licensee during this inspection. \302\267 Fireplace is properly screened to prevent access by children. \302\267 All hazardous items are not stored inaccessible to children. During facility tour, LPA observed two knives in the open dishwasher in the on-limits kitchen area and several poisons/toxins underneath the sink and in the on-limits restroom. \302\267 Weapons are not present. Licensee understands all guns, weapons and ammunition must be key locked separately and made inaccessible per Title 22 Regulations. \302\267 Facility is a one story home. \302\267 Verification of control of property on file. \302\267 Facility Sketch, Emergency Disaster Plan & Notification of Parent\342\200\231s Rights poster are posted. \302\267 Mandated Reporter Training expires on 06/2024. \302\267 Pediatric CPR and First Aid Card expires on 06/2024. \302\267 Health & Safety Certificate - completed on 03/17/2020. \302\267 No bodies of water at this time. Licensee understands all bodies of water including ponds, above ground pools & spas, in-ground pools & spas, and some fountains must be properly covered or fenced per Title 22 Regulations. The Department must be notified before and after installation of the above types of bodies of water. In addition, all wading pools or similar product must be emptied immediately after use and stored in an upright position. \302\267 Clean, safe and age appropriate toys \302\267 Current roster not on file. During record review, LPA observed that the roster presented to LPA was not completed. \302\267 Documentation of fire and disaster drills on file \342\200\223 Last drill conducted on 03/01/2024. \302\267 Children\342\200\231s records are complete. \302\267 The Licensee was informed of their reporting requirements and is provided with the Regional Office\342\200\231s Unusual Incident Reporting email mailbox: UnusualIncidentReportsDO09@dss.ca.gov \302\267 Resident and/or staff records reviewed on 05/08/2024 indicate that all adults who require caregiver background checks have not received all required clearances or exemptions. Upon arrival at the facility LPA observed an adult, S1, present at the home working with day-care children. S1 was not associated to the facility and had been working at the facility for approximately a year according to the Licensee. \302\267 The Licensee can submit transfer forms to associate new individuals or to disassociate someone from your facility at: Associations_Disassociations862@dss.ca.gov During facility tour, LPA observed several cribs/play pens present with non-tight fitting sheets and with several loose articles inside. LPA did not observe children asleep in the crib/play pen. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. LPA Perla Ordones informed licensee Geno Randle that this report dated 05/08/2024 document(s) 05/08/2024 Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA Perla Ordones informed the licensee Geno Randle to provide a copy of this licensing report dated 05/08/2024 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. A notice of site visit was given and must remain posted for 30 days.Failure to comply with posting requirements shall result in an immediate civil penalty of $100. During the exit interview, the LICENSEE Geno Randle confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.
